On January 28, 1986, NASA's Space Shuttle Challenger disintegrated 73 seconds after launch, resulting in the deaths of seven astronauts. Engineers at Morton Thiokol had raised concerns about the integrity of the O-rings used in the booster engines, warning that they might not function properly in cold temperatures. Despite these warnings, management proceeded with the launch, prioritizing commercial interests over safety.
